This takes a few minutes. Everything here is something the agreement requires before we can put a crew on your roof — your licence, your insurance, and who signs for your company.

This goes both ways. We ask for your licence and insurance because you are putting us on your customer's roof — and we send you ours for the same reason. See what we send you before you fill this in.

Insurance

Per §10, certificates are exchanged before anyone sets foot on site, and your general liability policy names us as additional insured. We also need to see that the policy carries no height or roof-work exclusion. Send the certificate to (619) 415-5514 or bring it to the signing call — we do not take file uploads on this form.

My policy carries a blanket additional-insured endorsement (automatic status where a written contract requires it), so this does not need re-issuing job by job. Tick only if you know. We will read the endorsement either way.

And the same in reverse. We name your company as additional insured on our general liability for liability arising out of our work, and we send you the endorsement with the certificate. Here is everything we send you.
